 ABOUT THE BREED:
As more and more people live in the city, needs have changed. Most are not looking for dogs who can hunt, we are looking for companions. We look for dogs who are playful, friendly towards people and pets, easy to train and easy to maintain.

In order to meet your expectations and make sure of the happiness of the puppy and your family, we'd be grateful for your answers to all of the questions below. If you have any comments or questions, please enter them in the "Comments & Questions" box at the end of the form.

1.  How many dogs have you (and your spouse or significant other) brought up? Please tell us about your experiences with raising puppies and having dogs in your life. Are you aware of what it takes to house-train and obedience train a puppy/dog?  Have you ever given away a dog you owned?

2.Do you have children? If yes, list ages.

3.Are your children (if any) good with pets?

4.How long have you been at your present address?

5.If you are renting, please give name, address and phone number of landlord.

6.Name address and phone number of your veterinarian.

7.Where will this puppy sleep?

8.Who will be the primary care giver for the puppy/dog?
9.Is someone home during the day or able to get home to feed/exercise the puppy?

10.How many hours on the average will the puppy be left alone?
11.Where will the puppy spend the day?

12.Will this puppy have regular contact with its family during the evenings?

13.How much indoor and outdoor room do you have for a dog to play? Do you have a fenced yard with your house or condo? If you do not have a fenced yard, how do you plan to keep your puppy/dog safe?

14.Will taking care of your family and/or children and your other daily responsibilities make it hard for you to attend to the puppy's house training, obedience training and attention/care needs?

15.Will your lifestyle allow you to give the puppy/dog the necessary attention and exercise the dog needs?

16.Are you willing to attend training classes?

17.Will the puppy be living with any children/people with allergies? If so, describe the type of allergies.

18.If you find out later that bringing a puppy/dog into your home was a mistake (for any reason) will you agree to contact us before taking any other action to sell or place the dog in another home?

19.The goldendoodle is a hybrid dog and as such, the puppies are not always standard with regard to specific size, coat type, weights, and personality characteristics. Are you aware of this and are you willing to agree to a puppy that may grow to the weights within these ranges: standard/large 55-70 pounds and standard/medium 40-55 pounds?

20.Please list the name and phone number of a personal reference that we can contact.

Please look back and make sure you have answered all the questions in the above form.
